Рефрактерными следует считать ПРК, которое требует использования вмешательств второй линии, включая назначение трех утеротоников и более, дополнительных препаратов, гемотрансфузию, хирургические методы гемостаза. Цель. Оценить состояние системы гемостаза у пациенток с рефрактерными ПРК при беременности, в родах и послеродовом периоде. Материалы и методы. В исследование включено 55 пациенток с рефрактерными ПРК и 165 пациенток без ПРК. Уровень гемоглобина, тромбоцитов и гематокрит определяли на гематологическом анализаторе Mindray BC-6800 Рlus на основе технологии SF Cube, уровень фибриногена, АЧТВ, ПТИ, МНО − на автоматическом анализаторе гемостаза SYSMEX CS-1600 на основе технологии мультиволнового анализа. Тромбоэластография проводилась на тромбоэластографе «TEG 5000» (Италия) c оценкой стандартных параметров ТЭГ, тестов EXTEM и FIBTEM. Объем кровопотери оценивали гравиметрическим методом. Анализ полученных данных и их визуализация проводились с применением языка программирования Python версии 3 и библиотек pandas, scipy.stats, matplotlib.pyplot. Результаты. У пациенток с ПРК при беременности перед родоразрешением регистрируется статистически значимо более низкий показатель фибриногена по сравнению с группой II − 3,9 (3,3; 4,5) и 4,4 (3,9; 4,96) соответственно (р = 0,002); в родах, несмотря на проводимую терапию, более низкие показатели − Hb (р = 0,010); Эр (р = 0,043) и фибриногена (р = 0,003); в послеродовом периоде – более низкие показатели Hb (p &lt; 0,001), эритроцитов (p &lt; 0,001), Ht (p &lt; 0,001), АЧТВ (р = 0,041), Тр (p &lt; 0,001) и фибриногена (р = 0,002). При оценке стандартных тестов ТЭГ у пациенток I группы относительно пациенток II группы в тесте EXTEM зарегистрированы статистически значимые отличия в тестах А5, MCF, ML; в тесте FIBTEM СТ в I группе был статистически значимо выше, чем во II группе; А5 и А10, напротив, ниже относительно группы II. Заключение. Пациентки с рефрактерными ПРК имеют нарушения в системе гемостаза, которые при беременности характеризуются более низкими показателями фибриногена. Нарушения гемостаза усугубляется при возникновении ПРК и прогрессируют к первым суткам послеродового периода</p></abstract><trans-abstract xml:lang="en"><p>Aim. To study the types of vaginal microbial communities in women with high-grade squamous intraepithelial lesion (HSIL) and those negative for intraepithelial lesion or malignancy (NILM). Materials and Methods. Between 2021 and 2023, we conducted an analysis of vaginal microbial community types among 70 women of reproductive age infected with high-risk oncogenic HPV: those with HSIL verified by cytological examination (n = 40) and those without precancerous cervical lesions (i.e., NILM, n = 30). Identification of microorganisms in the cervical mucus was identified by mass spectrometry. Results. The frequency of normal microbiota detected by the microscopic examination was 5.3-fold lower, whilst Streptococcus spp. was detected 3-fold times higher in women with HSIL compared to the NILM group (p = 0.023). Among patients with HSIL, community state type (CST) IV was the most frequent (75%), and CST IV-C1 subtype (with Streptococcus spp. as a prevailing genus) was found in 25% of cases (p = 0.132). In patients with NILM, CST IV was also the predominant type (60%) but CST IV-C2 (with Enterococcus spp. as a prevailing genus) was most frequent subtype (23.3%, p = 0.087). The prevalence of HPV type 16 and grade 2 abnormal colposcopy findings were 3.9-fold (p = 0.008) and 3.5-fold (p = 0.040) higher in women with HSIL than in NILM (p = 0.008). Conclusion.
