Во многих странах применяются программы организованного скрининга, включающие в основном цитологическое исследование. Однако данные многочисленных исследований показали, что подобная стратегия является низкоэффективной. Жидкостное цитологическое исследование позволяет уменьшить число неадекватных результатов, но также имеет низкую чувствительность. В настоящее время в программы организованного скрининга широко внедряется более эффективная методика - выявление ДНК вируса папилломы человека высокого онкогенного риска (ВПЧ-ВР), который присутствует почти в 100% случаев РШМ. Для эффективного скрининга необходим не только чувствительный метод диагностики, но и широкий охват женского населения существующей SHAPE \* MERGEFORMAT программой. Среди респондентов установлено, что основными причинами игнорирования скрининга РШМ являются дискомфорт во время гинекологического осмотра, страх положительного результата и отсутствие свободного времени. Именно для такой категории пациенток разработаны устройства для самостоятельного взятия вагинального отделяемого, которое в дальнейшем будет исследовано на наличие ДНК ВПЧВР. Существуют различные модификации таких приспособлений, но основная цель - увеличение охвата населения скринингом рака шейки матки. При наличии положительного результата такого скринингового теста женщина будет приглашена и обследована в условиях гинекологического кабинета. Доказано, что результаты ВПЧ-тестирования после взятия материала врачом и самостоятельного забора образца сопоставимы и являются более чувствительным методом, чем цитологическое исследование.</p></abstract><trans-abstract xml:lang="en"><p>The identification of cerval intraepithelial neoplasia is of crucial importance for reducing both incidence and mortality of cervical cancer. Currently, screening programs relying on Papanicolaou test or liquid-based cytology are employed in many countries but have a limited efficiency. Recently, HPV genotyping was suggested as a possible screening alternative as high-risk HPV strains are detected in almost all cervical cancer biopsies. Importantly, in addition to high sensitivity, efficient screening approach also requires a wide population coverage. As the main reasons for cervical cancer screening avoidance include fear of examination, embarrassment, and lack of time, self-sampling devices have the potential to significantly improve screening coverage. Such an approach implies invitation for gynecological examination only if high-risk HPV strains would be detected in the self-collected sample.
