Представить клинический случай наблюдения и лечения пациентки с сахарным диабетом 1 типа с подозрением на нейроэндокринную опухоль поджелудочной железы (клинически ВИПома).</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Описание клинического случая тяжелого течения сахарного диабета 1 типа.</p></sec><sec><title>Результаты</title><p>Результаты. Гастроинтестинальная форма диабетической автономной невропатии проявляется гипо- и атонией пищевода (у 2–27 % больных сахарным диабетом (СД) и желудка (7–29 %), гипомоторикой кишечника, диареей (0–22 %), атонией желчного пузыря и дискинезией желчных путей со склонностью к холелитиазу, реактивным панкреатитом. Имеется прямая зависимость частоты развития диабетической автономной невропатии от длительности и степени компенсации СД. Диабетическая диарея возникает в результате нарушения иннервации, синдрома избыточного бактериального роста, недостаточности внешнесекреторной функции поджелудочной железы, нарушения обмена желчных кислот вследствие дискинезии желчевыводящих путей, нарушения моторики толстой кишки, аноректальной дисфункции, наличия микроангиопатии и атрофии слизистой оболочки кишечника, витаминной недостаточности. Распространенность диареи у больных СД колеблется в пределах 4–22 %. В 20-44% диабетическая диарея в рамках автономной гастропатии может чередоваться с запорами. Своевременное лечение гастроинтестинальных осложнений СД может увеличить продолжительность активной жизни больных. Основной целью терапии диабетической гастропатии являются компенсация уровня гликемии и уменьшение диспепсической симптоматики. Компенсация уровня гликемии является решающим действием в профилактике развития автономной гастропатии.</p></sec><sec><title>Заключение</title><p>Заключение. Пациенты с сахарным диабетом должны достигать целевых значений углеводного обмена и избегать гипогликемий для профилактики развития диабетических осложнений. Автономная гастропатия представляет собой трудности в ведении больного для практикующего врача и значительно снижает качество жизни пациента. Нейроэндокринная опухоль (ВИПома) может быть заподозрена при длительной интерметтирующей диарее.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To present a clinical case of observation and treatment of a patient with type 1 diabetes mellitus and suspected pancreatic neuroendocrine tumor (clinically VIPoma).</p></sec><sec><title>Materials and Methods</title><p>Materials and Methods. Description of a clinical case of severe type 1 diabetes mellitus.</p></sec><sec><title>Results</title><p>Results. The gastrointestinal form of diabetic autonomic neuropathy is manifested by hypo- and atony of the esophagus (in 2-27% of patients with diabetes) and stomach (7-29%), intestinal hypomotility, diarrhea (0-22%), gallbladder atony and biliary dyskinesia with a tendency to cholelithiasis and reactive pancreatitis. There is a direct relationship between the incidence of diabetic autonomic neuropathy and the duration and degree of compensation of diabetes. Diabetic diarrhea results from impaired innervation, bacterial overgrowth syndrome, pancreatic insufficiency, bile acid metabolism disorders due to biliary dyskinesia, impaired colonic motility, anorectal dysfunction, microangiopathy and atrophy of the intestinal mucosa, and vitamin deficiency. The prevalence of diarrhea in patients with diabetes ranges from 4% to 22%. In 20-44% of cases, diabetic diarrhea associated with autonomic gastropathy may alternate with constipation. Timely treatment of gastrointestinal complications of diabetes can increase the active life expectancy of patients. The primary goals of diabetic gastropathy therapy are glycemic control and reduction of dyspeptic symptoms. Glycemic control is crucial in preventing the development of autonomic gastropathy.</p></sec><sec><title>Conclusion</title><p>Conclusion. Patients with diabetes mellitus should achieve target carbohydrate metabolism values and avoid hypoglycemia to prevent the development of diabetic complications. Autonomic gastropathy presents challenges in patient management for the practitioner and significantly reduces the patient's quality of life.
