Изучение структуры внутренней картины болезни у ВИЧ-инфицированных пациентов в зависимости от клинико-эпидемиологических характеристик.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы: Обследовано 86 ВИЧ-инфицированных пациентов в возрасте от 19 до 59 лет с различным сроком инфицирования и тяжестью течения болезни. Пилотное исследование выполнено на базе ГБУЗ «Кузбасский Центр-СПИД». Использованы: «Краткий опросник восприятия болезни» E. Broadbent в русскоязычной адаптации; «Личностный смысл болезни» (Z.L. Lipowski); «Опросник эмоционального реагирования на болезнь»; t-критерий Стъюдента для независимых выборок; дескриптивная статистика; кластерный анализ; факторный анализ осуществлялся методом главных компонент посредством вращения Varimax c учетом теста Кайзера. В рамках проведенного исследования систематизированы результаты исследований субъективного реагирования личности на ВИЧ-инфекцию как хроническую неизлечимую жизнеугрожающую болезнь.</p></sec><sec><title>Результаты исследования</title><p>Результаты исследования: Внутренняя картина болезни у ВИЧ-инфицированных пациентов определяется тяжестью течения болезни. Срок инфицирования согласован с когнитивной оценкой угрозы болезни и оказывает менее заметное влияние на характер субъективного реагирования на болезнь. Когнитивная оценка болезни согласована с тяжестью течения ВИЧ-инфекции. У ВИЧ-инфицированных пациентов в состоянии ремиссии со сроком инфицирования более 10 лет доминирует когнитивный уровень ВКБ. Эмоциональные репрезентации болезни амбивалентны (самостигматизация сочетается с надеждой, оптимизмом и радостью от преодоленных трудностей). Мотивационно-смысловой уровень внутренней картины болезни (ВКБ) характеризуется преобладанием позитивных личностных смыслов. У пациентов с прогрессирующими формами ВИЧ-инфекции независимо от срока инфицирования все компоненты ВКБ соответствуют оценке – «болезнь – выраженная актуальная угроза жизни и здоровью» и отражают острое стрессогенное влияние болезни. Эмоциональный уровень ВКБ депрессивен (отчаяние и беспомощность – ведущие эмоции); доминируют негативные личностные смыслы («болезнь как угроза, потеря здоровья, слабость»).</p></sec><sec><title>Заключение</title><p>Заключение. Тяжесть течения заболевания и когнитивная оценка ВИЧ-инфекции оказывают влияние на специфику развития различных вариантов внутренней картины болезни. Своеобразие адаптации к болезни лиц, живущих с ВИЧ (ЛЖВ) в состоянии ремиссии (как высоко приверженных АРТ) с неопределяемой вирусной нагрузкой и длительным стажем болезни заключается в том, что ведущим адаптационным ресурсом в структуре ВКБ является убежденность в контролируемости болезни.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. Study the Structure of the Internal Picture of Disease in HIV-Infected Patients Depending on Clinical and Epidemiological Characteristics.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. 86 HIV-infected patients aged 19 to 59 years were examined, with varying duration of infection and severity of disease progression. The pilot study was conducted at the Kuzbass AIDS Center. Research methods: questionnaire "Short questionnaire of E. Broadbent disease perception in Russian adaptation; "Personal meaning of the disease" (Z.L. Lipowski); "Emotional Response Questionnaire"; Student's t-test for independent samples; descriptive statistics; factor analysis was performed by principal component method by Varimax rotation taking into account Kaiser test; cluster analysis. As part of the study, the results of studies of the subjective response of the individual to HIV infection as a chronic incurable life-threatening disease are systematized.</p></sec><sec><title>Results</title><p>Results. The study systematized research findings on subjective responses to HIV infection as a chronic, incurable, life-threatening disease. The internal picture of disease in HIV-positive patients is determined by disease severity. The duration of infection correlates with cognitive assessment of disease threat but has less impact on subjective response. Cognitive assessment aligns with HIV infection severity. In patients in remission with over 10 years of infection, the cognitive level of the internal picture of disease (IPD) dominates. Emotional representations show ambivalence (self-stigmatization combined with hope, optimism, and joy from overcoming difficulties). The motivational-meaning level of IPD is characterized by predominance of positive personal meanings. In patients with progressive forms of HIV infection, regardless of infection duration, all IPD components reflect acute stress impact. Emotional level of IPD shows depression (despair and helplessness as leading emotions). Negative personal meanings dominate («disease as threat», loss of health, weakness).</p></sec><sec><title>Conclusion</title><p>Conclusion. The severity of disease progression and cognitive assessment of HIV infection influence the development of different variants of the internal picture of disease.
