Причинами появления грыж являются врождённая или приобретённая несостоятельность соединительной ткани, возрастная дисплазия мышц, отсутствие облитерации дивертикулярных выбуханий париетальной брюшины у плода, формирование предбрюшинных липом как предпосылок их смещения в грыжевые ворота, врождённое широкое бедренное кольцо или его расширение после родов и натяжных герниопластик паховых грыж, подтягивающих вверх паховую связку. Физикальное обследование выявляет грыжи в 93%, осмотр предбрюшинного пространства ‒ в 100%. В сомнительных случаях для диагностики рекомендуется использовать УЗИ, КТ, МРТ, герниографию. Во второй половине 19-го века была разработана единая концепция лечения паховых грыж, имеющая под собой анатомический фундамент и предусматривающая вскрытие пахового канала, высокую перевязку грыжевого мешка, пластику задней или передней стенки пахового канала. Для бедренных грыж базовыми открытыми операциями стали способы закрытия грыжевых ворот со стороны бедра и со стороны пахового канала. В первой половине 20-го века начали развиваться задние подходы для закрытия грыжевых ворот из надлобкового (надлонного) и надпаховых доступов без вскрытия пахового канала. Внедрение синтетических сетчатых протезов привело к распространению ненатяжных открытых герниопластик передними и задними доступами. С появлением современной эндоскопической техники начали выполнять малоинвазивные видеолапароскопические (ТАПП) и преперитонеоскопические (ТЭП) герниопластики с размещением сетки в пространстве между брюшиной и поперечной фасцией, закрывая входы в паховый канал и бедренное кольцо. Современные технологии сократили сроки лечения, нетрудоспособности и снизили рецидивы заболевания до 1,4 – 2%.</p></abstract><trans-abstract xml:lang="en"><p>Groin hernias occur in 27–48% of men and 3–6% of women during lifetime. Among the causes are inherited or age-related connective tissue or muscular dysplasia, lack of diverticular obliteration in fetal parietal peritoneum, pre-peritoneal lipomas migrating into the hernia ring, and congenital, postpartum, or postoperation dilation of the femoral ring. Physical examination and examination of the preperitoneal space reveals hernia in 93% and 100%, respectively, yet ultrasound examination, computed tomographic peritoneography, magnetic resonance imaging are also employed to confirm the diagnosis. An established technique for the treatment of groin hernias includes opening the inguinal canal, high ligation of the hernia sac, and repair of the inguinal canal, altogether termed herniorrhaphy. Suprapubic or inguinal incision permits closure of the hernia sac without involving the inguinal canal (herniotomy). Implementation of synthetic meshes led to the increase in hernioplasty, i.e. reinforcement of the inguinal canal upon the inguinal canal repair. Laparoscopic transabdominal preperitoneal (TAPP) and totally extraperitoneal (TEP) hernioplasty became widely used along with the development of endoscopic surgery. In these techniques, mesh is placed between the peritoneum and transversalis fascia, closing both inguinal canal and femoral ring.
