Оценка эффективности ингибитора IL-6 (олокизумаба) в подавлении гипервоспаления и улучшении исходов лечения пациентов со среднетяжелой пневмонией, обусловленной штаммом SARS-CoV-2.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Сплошным методом были отобраны 48 пациентов с подтвержденной коронавирусной инфекцией, имеющие показания для назначения олокизумаба, были разделены на 2 группы. В группу А (n=29) включались пациенты, получавшие олокизумаб в дозе 160 мг/мл - 0,4 мл подкожно однократно, в группу В (n=19) включались пациенты, не получавшие препарат из-за наличия противопоказаний для его введения. Первой контрольной точкой для получения лабораторных показателей являлись первые сутки накануне введения олокизумаба, второй контрольной точкой – пятые сутки после введения препарата.Первичным исходом исследования являлись исходы лечения больных со среднетяжелой пневмонией, обусловленной коронавирусной инфекцией, вторичным исходом исследования являлось влияние олокизумаба на лабораторные критерии цитокинового шторма.</p></sec><sec><title>Результаты</title><p> Результаты. Во второй контрольной точке исследования показатели SpO2 составили 90% (88;92) в группе А и 90% (88;93) в группе В, U [29;19]=220,5, р=0,99; на ИВЛ находились 0% и 21,1% пациентов соответственно, р=0,001; лабораторные критерии цитокинового шторма соответственно составили: С-реактивный белок 5,6 (1,8;13,5) и 23 (17;92,5) мг/л (U[29;19]=61, р=0,0002), ферритин 325 (200;700) и 215 (186;526) нг/мл (U[29;19]=119,5, р=0,41), D-димер 827 (378;1940) и 1464 (677;4600) нг фибриноген-экв. ЕД/мл (U[29;19]=197,5, р=0,16), лейкоциты 5,9 (4,8;6,2) и 7,6 (6,6;12)*109 /л (U[29;19]=162,5, р=0,03), лимфоциты 1,3 (0,8;2,1) и 1,5 (1;2,1)*109 /л (U[29;19]=225, р=0,44). Выздоровление зарегистрировано у 96,5% пациентов в группе А и у 78,9% в группе В, р=0,05; смерть произошла у 3,5% и у 21,1% пациентов соответственно, р=0,05.</p></sec><sec><title>Заключение</title><p> Заключение. В настоящем исследовании олокизумаб продемонстрировал способность снижать клинические проявления (гипертермию) и некоторые биохимические предикторы цитокинового шторма: СРБ и уровень лейкоцитов. Также при введении препарата были выявлены положительные клинические эффекты: снижение доли пациентов, нуждающихся в проведении ИВЛ, и большая частота положительных исходов лечения.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To study the efficiency of IL-6 inhibitor olokizumab in suppressing the inflammation and improving treatment outcomes in patients with moderate COVID-19 pneumonia.</p></sec><sec><title>Materials and Methods</title><p> Materials and Methods. We enrolled 48 patients with confirmed COVID-19 pneumonia who had indications for the treatment with olokizumab. Treatment group (n = 29) included patients who received a single subcutaneous injection of olokizumab (0.4 mL, 160 mg/mL) while placebo group (n = 19) included those who did not receive the drug because of contraindications. Inflammatory markers (C-reactive protein, white blood cell count, lymphocyte count, ferritin and D-dimer) have been evaluated on the 1st and the 5th day after the administration of olokizumab. Efficiency of olokizumab was assessed by the treatment outcome and suppression of cytokine storm.</p></sec><sec><title>Results</title><p> Results. At the 5th day, SpO2 was 90% (88; 92-93) in both groups, yet none of the patients in the treatment group required a mechanical ventilation, in contrast to 21.1% patients in the placebo group (p = 0.001). Inflammatory parameters were also reduced in the treatment group according to the measurements of serum C-reactive protein (5.6 (1.8;13.5) and 23 (17;92.5) mg/L in the treatment and placebo group, respectively, p = 0.0002) and white blood cell count (5.9 (4.8;6.2) and 7.6 (6.6;12 )*109 /L in the treatment and placebo group, respectively, p=0.03). No statistically significant differences were registered regarding serum ferritin (325 (200; 700) and 215 (186; 526) ng/mL, p = 0.41), D-dimer (827 (378; 1940) and 1464 (677; 4600) ng FEU/mL, p = 0.16) and lymphocyte count (1.3 (0.8; 2.1) and 1.5 (1; 2.1)*109 /L, p =0.44). Death was registered in 3.5% and 21.1% of patients in the treatment and placebo group, respectively, (p = 0.05).</p></sec><sec><title>Conclusion</title><p> Conclusion.
