Первые сообщения о высокой эффективности РД дали толчок к развитию технологии. Однако после публикации данных первого рандомизированного контролируемого клинического исследования (РКИ) Symplicity HTN-3, не показавшего преимуществ технологии, интерес к ней значительно угас. Разработка и публикации данных новых РКИ с усовершенствованным дизайном SPYRAL HTNOFF MED и SPYRAL HTN-ON MED позволили методу возродиться. В исследованиях было показано, что РД приводит к снижению как офисных, так и среднесуточных значений АД, эффективна у пациентов с неконтролируемой АГ даже в условиях отсутствия медикаментозной терапии в сравнении с ложной (sham) процедурой. Кроме того, эффект вмешательства сохраняется в течение 24 часов, характеризуется положительным влиянием на параметры суточного профиля АД и не зависит от приверженности к лечению. В настоящем обзоре представлены данные об эффективности и безопасности технологии, отражены ее органопротективные эффекты, а также обсуждается вопрос о преимуществах РД у различных групп пациентов.</p></abstract><trans-abstract xml:lang="en"><p>Renal denervation (RDN) is a method of the sympathetic nervous system modulation which has been specifically developed for the treatment of resistant hypertension. Initial reports demonstrating high efficiency of RDN in such patients propelled the development of the technology, yet a specific randomised controlled trial Symplicity HTN-3 did not show its benefits. Data from the subsequent RCTs SPYRAL HTN-OFF MED and SPYRAL HTN-ON MED have shown that RDN reduces both office and daily blood pressure, does not depend on the patient adherence, and is efficient in patients with uncontrolled hypertension even in the absence of drug therapy.
