Изучить отдалённые изменения когнитивного статуса пациентов с сахарным диабетом (СД) 2-го типа через 5−7 лет после проведения коронарного шунтирования (КШ).Материалы и методы. В исследование были включены 47 пациентов мужского пола, поступивших в кардиологическое отделение для проведения коронарного шунтирования. Критерии включения: подписанное добровольное информированное согласие, возраст от 40 до 75 лет, мужской пол, планируемое КШ в условиях искусственного кровообращения (ИК), праворукость. Критерии исключения: патологические изменения головного мозга по результатам мультиспиральной компьютерной томографии (МСКТ), хроническая ишемия головного мозга выше II степени, сумма баллов по шкале Бека более 16 баллов, по шкале Mini-Mental State Examination (MMSE) менее 24 баллов и шкале Frontal Assesment Battery (FAB) менее 11 баллов, жизнеугрожающие нарушения ритма и проводимости, хроническая сердечная недостаточность (ХСН) II Б стадии и выше, хроническая обструктивная болезнь лёгких, онкопатология, заболевания и травмы головного мозга, любые эпизоды нарушения мозгового кровообращения в анамнезе. По результатам предоперационного обследования пациентов были выделены две группы: с наличием СД 2-го типа (n=21) и его отсутствием (n=26).Результаты. Обнаружено, что через 5−7 лет после КШ у пациентов с СД 2-го типа произошло снижение когнитивного статуса по шкале MMSE по сравнению с предоперационным уровнем (28,0 [27,0; 29,0] и 27,0 [26,0; 28,0], p=0,04). При этом при развитии умеренного когнитивного расстройства (УКР) у пациентов с СД 2-го типа отношение шансов (ОШ) составило 1.92 (95 % ДИ=1,09-3,37; Z=2,26, p=0,02). Пациенты с СД 2-го типа имели худшие показатели психомоторных и исполнительных функций (нейродинамика) как исходно, так и через 5−7 лет после КШ (p≤0,05). Через 5−7 лет после КШ только у пациентов с СД 2-го типа обнаружены корреляции между уровнем гликированного гемоглобина (HbA1c) и когнитивными показателями. Чем выше был уровень HbA1c, тем хуже были скоростные характеристики тестов исполнительных функций и показатели кратковременной памяти.Заключение. Сохранение негативных изменений психомоторных и исполнительных функций у пациентов с СД 2-го типа продемонстрировано через 5−7 лет после проведения КШ. Вероятность снижения общего когнитивного статуса по сравнению с предоперационным уровнем у пациентов с СД 2-го типа была в 1,92 раза выше, чем у пациентов без СД. Уровень HbA1c имеет негативные ассоциации с показателями психомоторных и исполнительных функций в отдалённом периоде КШ.</p></abstract><trans-abstract xml:lang="en"><p>Aim. To study long-term changes (5 to 7 years after coronary artery bypass graft (CABG) surgery) in cognitive status of patients with type 2 diabetes.Materials and Methods. The study included 47 male patients admitted to the Research Institute for Complex Issues of Cardiovascular Diseases for the CABG surgery. Criteria of inclusion were signed informed consent, age from 40 to 75 years, male gender, planned on-pump CABG surgery, and right-handedness. Criteria of exclusion were brain pathology at multislice computed tomography, chronic cerebral ischemia grade II-IV, Beck Depression Inventory score &gt; 16 points, the Mini-Mental State Examination (MMSE) score &lt; 24 points and the Frontal Assessment Battery score &lt; 11 points, arrhythmia, class IIB-IV chronic heart failure stage IIB, chronic obstructive pulmonary disease, cancer, and past medical history of brain injury or stroke. Pre-operative examination of patients classified them into two groups: with (n = 21) and without (n = 26) type 2 diabetes mellitus (DM).Results. 5-7 years after CABG surgery, patients with type 2 DM had a cognitive decline according to MMSE scale as compared to the preoperative level (28,0 [27,0; 29,0] and 27,0 [26,0; 28,0], p = 0.04). In keeping with these findings, odds ratio (OR) of mild cognitive impairment (MCI) in patients with type 2 DM was 1.92 (95% CI = 1.09- 3.37, p = 0.02). Psychomotor and executive functions were reduced in patients with type 2 DM both at baseline and in particular 5-7 years after CABG (p ≤ 0.05). The correlation between glycated hemoglobin (HbA1c) and cognitive parameters were found only in patients with type 2 DM.
