Цель. Изучение особенностей внутренней картины болезни в сочетании с уровнем комплаентности больных хроническим гастритом.Материалы и методы. Обследовано 302 пациента с хроническим гастритом (ХГ). В зависимости от уровня приверженности лечению пациенты включены в 4 группы: группа 1 − 96 пациентов с ХГ, ассоциированным с Helicobacter pylori (HP), группа 2 − 110 пациентов с ХГ, не ассоциированным с HP (группы с высоким уровнем комплаентности), группа 3 − 36 пациентов с ХГ, ассоциированным с HP, которые отказались от лечения, группа 4 − 60 пациентов с ХГ, ассоциированным с HP, которые частично выполняли рекомендации врача. Уровень приверженности обследуемых проводимому лечению оценивали по опроснику «Уровень комплаентности» Давыдова, личностное реагирование на болезнь − с помощью опросника ТОБОЛ. Поиск взаимосвязей исследуемых показателей проводили с использованием корреляционного анализа.Результаты. Установлено, что максимальная степень эрадикации возбудителя ХГ наблюдалась у больных с высокими уровнями приверженности лечению, минимальный результат лечения выявлен у больных с низким уровнем комплаентности. У большинства пациентов (56,0%) установлены адаптивные типы отношения к заболеванию, значительно реже − смешанные типы (18,2%), у 12,5% − дезадаптивные типы с интрапсихической направленностью и в 11,6% случаях − дезадаптивные типы с интерпсихической направленностью. В группе 3 среди «некомплаентных» обследуемых статистически значимо реже, чем в группах 1 и 2, выявлялись адаптивные типы, в то же время в третьей группе было значительно больше пациентов с анозогнозическим и дезадаптивным типами отношения к заболеванию с интрапсихической направленностью. Корреляционный анализ показал, что уровень приверженности лечению больных с ХГ положительно коррелирует с частотой выявления гармоничного типа отношения к заболеванию, а также с эргопатическим типом реагирования. У некомплаентных пациентов более высокой является частота анозогнозического типа отношения к заболеванию.Заключение. Тип отношения пациента с хроническим гастритом к болезни существенно влияет на его комплаентность, этот фактор следует учитывать при выборе тактики лечения.

Aim. To study the associations between the attitude towards illness and the level of compliance in patients with chronic gastritis.Materials and Methods. The study included 302 patients with chronic gastritis which have been stratified into 4 groups: 1) 96 patients with Helicobacter pylori (HP)-associated chronic gastritis and high treatment adherence; 2) 110 patients with HP-negative chronic gastritis and high treatment adherence; 3) 36 patients with HP-associated gastritis who refused treatment; 4) 60 patients with HP-associated gastritis and moderate treatment adherence. The level of adherence to treatment and personal response to the disease was assessed using Davydov and TOBOL questionnaires, respectively. Interrelations of the studied parameters were evaluated using correlation analysis.Results. Eradication of HP was associated with a high treatment adherence. The majority of patients (56.0%) had adaptive behavioural response, whilst mixed (18.2%) and maladaptive types (12.5% for intrapsychic and 11.6% for interpsychic orientation) were less frequently observed. In patients who refused treatment, adaptive behavioural response was less common than anosognosic and maladaptive attitudes. Correlation analysis revealed that treatment adherence positively correlates with harmonious and ergopathic attitudes.Conclusion. Attitude to illness affects adherence to treatment in patients with chronic gastritis.
