Изучение эффективности и отдалённых результатов органосберегающего оперативного лечения пациенток с тяжёлыми формами АМ, находящихся в позднем репродуктивном и перименопаузальном возрасте.Материалы и методы. Объект исследования − 18 пациенток с тяжёлыми формами диффузного АМ, которым было проведено оперативное лечение в объеме аденомиомэктомии по методике Х. Осада. Эффективность оперативного лечения и последующей медикаментозной терапии оценивалась в ходе динамического наблюдения за пациентками через 12 и 24 месяца. Была проведена сравнительная оценка размеров и объёма матки, полученных в результате ультразвукового исследования, а также частоты симптомов АМ и оценки качества жизни путем использования стандартного опросника SF-36.Результаты. В процессе наблюдения за пациентками было отмечено восстановление и нормализация менструального цикла – в 100% случаев (18/18), исчезновение болевого синдрома – в 100% случаев (18/18), ликвидация анемии – 100% (18/18), наступление менопаузы в 50% случаев (9/18), улучшение качества жизни – в 100% случаев (18/18). Полученные результаты анализа данных опросника по оценке качества жизни SF-36, сгруппированные в показатель «физический компонент здоровья» и включавшие данные пациенток с АМ до оперативного лечения, а также через 12 и 24 месяца после оперативного, показали достоверное увеличение средних значений баллов по всем четырем шкалам.Заключение. Операция расширенной аденомиомэктомии по методике Х. Осада является операцией выбора и может быть альтернативой гистерэктомии не только у женщин с диффузными и смешанными формами АМ, не реализовавших свои репродуктивные планы, но и у женщин с АМ, находящихся в позднем репродуктивном и перименопаузальном возрасте.</p></abstract><trans-abstract xml:lang="en"><p>Aim. To study the short- and long-term results of organ-sparing surgery in patients with severe diffuse adenomyosis who are in the perimenopausal age.Materials and Methods. The study included 18 patients with severe diffuse adenomyosis who underwent fertility-sparing excision of diffuse adenomyosis by means of Osada procedure. Efficiency of surgical treatment and subsequent drug therapy was evaluated at 12 and 24 months of follow-up by pelvic ultrasound examination. The frequency of adenomyosis symptoms and quality of life were assessed using the standard SF-36 questionnaire.Results. Recovery of the menstrual cycle, freedom from pain and anemia, and improved quality of life were documented in all cases (18/18), whereas onset of menopause occurred in half of the cases (9/18). In particular, all scales of the Physical Component Summary from SF-36 questionnaire were significantly improved at both follow-up time points.Conclusion.
