Анализ особенностей клинической характеристики курящих больных с многососудистым поражением коронарного русла.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Проанализированы данные регистра FRAILTY, включающего пациентов с ишемической болезнью сердца (ИБС). Кроме клинико-анамнестических показателей оценены параметры ЭхоКГ, липидного обмена (общий холестерин, триглицериды, липопротеиды низкой и высокой плотности, уровень высокочувствительного С-реактивного белка).</p></sec><sec><title>Результаты</title><p>Результаты. Из 381 больного ИБС 178 (46,7%) отнесены к группе когда-либо куривших (107 (60,1%) – курящие в настоящее время, 71 (39,1%) – курившие ранее), 203 (53,3%) пациента никогда не курили. Для дальнейшего анализа сформированы две группы: курящие/курившие ранее и никогда не курившие пациенты. Группы были сопоставимы по возрасту и основной сердечно-сосудистой патологии. Когда-либо курившие больные характеризовались большей долей лиц мужского пола (78,1 против 69%; р = 0,045), чаще перенесенным инфарктом миокарда (61,8 против 55,2%; р = 0,047), большей частотой хронической обструктивной болезни легких (6,4 против 2,3%; р = 0,045), атеросклеротических заболеваний нижних конечностей и мультифокального атеросклероза (47,8 против 31,5% и 47,8 против 63,5% соответственно; р = 0,041–0,046). Не обнаружено различий по вероятному риску летального исхода после кардиохирургического лечения. Уровень общего холестерина был в 1,8 раза выше у когда-либо куривших в сравнении с никогда не употреблявшими табак лицами (5,5 (3,7; 6,7) против 3,0 (2,2; 4,0) ммоль/л; р = 0,01). Аналогичная закономерность выявлена в отношении уровня С-реактивного белка: по сравнению с никогда не курившими лицами его концентрация у курящих была в 1,3 раза выше (5,1 (2,4; 5,3) против 3,9 (2,2; 4,2) мг/л; р = 0,041). Анализ ЭхоКГ-параметров не продемонстрировал статистически значимых различий.</p></sec><sec><title>Заключение</title><p>Заключение. Определена высокая частота курения среди пациентов с ИБС. Курящие/курившие ранее больные характеризовались большей частотой ранее перенесенного инфаркта миокарда, хронической обструктивной болезни легких, атеросклеротических заболеваний нижних конечностей и мультифокального атеросклероза. Кроме того, курившие когда-либо пациенты имели более высокие концентрации общего холестерина и С-реактивного белка.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To analyse clinicopathological features of smoking patients with multivessel coronary artery disease.</p></sec><sec><title>Material and Methods</title><p>Material and Methods. We analysed the data from the FRAILTY registry which includes patients with coronary artery disease. In addition to clinical features, we assessed echocardiography data and lipid metabolism levels (total cholesterol, triglycerides, low- and high-density lipoproteins, and high-sensitive C-reactive protein).</p></sec><sec><title>Results</title><p>Results. Of the 381 CAD patients, 178 (46.7%) have smoked at some point (107 (60.1%) current smokers and 71 (39.1%) smoking quitters), whereas 203 (53.3%) patients have never smoked. These groups were comparable in age and cardiovascular disease prevalence. Smokers or smoking quitters’ group had higher proportion of males (78.1 vs. 69%; p = 0.045), higher prevalence of myocardial infarction (61.8 vs. 55.2%; p = 0.047), chronic obstructive pulmonary disease (6.4 vs. 2.3%; p = 0.045), and lower extremity peripheral artery disease (47.8 vs. 31.5%; p = 0.041). There were no differences between the groups regarding the risk of death after cardiac surgery. The level of total cholesterol was 1.8-fold higher in smokers and smoking quitters as compared with patients who never smoked (5.5 (3.7; 6.7) vs. 3.0 (2.2; 4.0) mmol/L; p = 0.01). Similar pattern was discovered regarding to the level of C-reactive protein (5.1 (2.4; 5.3) vs. 3.9 (2.2; 4.2) mg/L in smokers/smoking quitters and never smokers, respectively; p = 0.041). The analysis of echocardiography parameters did not reveal statistically significant differences.</p></sec><sec><title>Conclusion</title><p>Conclusion. Smoking frequently accompanies coronary artery disease.
