Диагностика аденомиоза зачастую затруднена ввиду сопутствующих гиперпластических процессов в матке. Морфологическим критерием аденомиоза является наличие островков эндометрия в мышечном слое матки. Патогенез данного заболевания еще не выяснен окончательно и требует проведения дальнейших фундаментальных и клинических исследований. К развитию аденомиоза приводит повышенная способность некоторых клеток эндометрия к инвазии в миометрий. Предполагается влияние местного и системного иммунитета на развитие и прогрессирование патологического процесса. Данными некоторых научных исследований подчеркивается роль активации иммунных клеток и высвобождение растворимых иммунных факторов, при этом активируются как провоспалительные, так и противовоспалительные компоненты иммунной системы. У женщин с аденомиозом наблюдается дисбаланс в системе интерферонов, повышение уровня некоторых ростовых факторов, дефенсинов и хемокинов. Выявлено, что степень иммунологических системных изменений пропорциональна тяжести клинических симптомов заболевания. Системные и местные иммунные нарушения влекут за собой неблагоприятные последствия для репродуктивной функции женщин. Так, возникающий при аденомиозе оксидативный стресс приводит к избытку свободных радикалов, которые могут повреждать эмбрион. Изменение концентрации молекул клеточной адгезии препятствует имплантации, а морфологические изменения миометрия ведут к нарушению его перистальтики и затрудняют транспорт сперматозоидов. Доказано, что изменения цитокинового профиля и уровня регуляторно-транспортных белков у женщин с аденомиозом могут служить предикторами исходов программ вспомогательных репродуктивных технологий. В данном обзоре представлены последние данные научной литературы об иммунологических изменениях при аденомиозе и их влиянии на репродуктивную функцию женщин.</p></abstract><trans-abstract xml:lang="en"><p>Adenomyosis, an ingrowth of endometrial tissue into the uterine wall, is a benign gynecological disease associated with infertility, pelvic pain, and abnormal uterine bleeding, having serious consequences for a woman's health and quality of life. Diagnosis of adenomyosis is often challenging due to the concomitant endometrial hyperplasia. The pathogenesis of adenomyosis has not yet been fully evaluated and requires further fundamental and clinical research. Here we analyze an impact of local and systemic immunity on the development and progression of adenomyosis. Several studies highlight the role of immune cell activation and excessive release of both proinflammatory and anti-inflammatory cytokines. Women with adenomyosis exhibit an imbalance in the interferon system, elevated levels of certain growth factors, defensins, and chemokines. Immune system disorders well correlate with the clinical symptoms of adenomyosis and entail adverse consequences for the female reproductive system. Oxidative stress that occurs in adenomyosis leads to an excess of free radicals that can damage the embryo. Alterations in cell adhesion levels prevent implantation, and morphological changes in the myometrium lead to a violation of its peristalsis and impede the sperm transport. Changes in the cytokine profile and the levels of transport proteins in women with adenomyosis may serve as predictors of the assisted reproductive technology outcomes.
