Определение частоты и факторов риска послеоперационной когнитивной дисфункции (ПОКД) у пациентов, имеющих предоперационное умеренное когнитивное расстройство (УКР) и перенёсших коронарное шунтирование (КШ).</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. До операции были сформированы две группы, включающие 50 больных с наличием и 51 с отсутствием УКР. Нейропсихологическое исследование (12 тестов) проводилось до КШ, на 10-е сутки и через 1 год. Ранняя (10-е сутки после операции) и отдалённая (через 1 год) ПОКД диагностировались при 20 % снижении показателей в 20 % тестов.</p></sec><sec><title>Результаты</title><p>Результаты. Пациенты с УКР до операции имели более низкий уровень образования (p = 0,03), более низкие показатели фракции выброса левого желудочка, (p = 0,02) и более тяжелое поражение коронарного русла по шкале SYNTAX, (p = 0,01). Факторами, значимыми для развития ранней ПОКД, явились продолжительность искусственного кровообращения (p = 0,035) и выраженность полиорганной недостаточности, оцененная по шкале SOFA (p = 0,04). Ранняя ПОКД у них выявлялась в 72 % случаев, отдалённая ПОКД – в 54 % случаев, а при развитии у этих пациентов ранней ПОКД – в 81 % случаев.</p></sec><sec><title>Заключение</title><p>Заключение. Хроническая сердечная недостаточность с низкими показателями фракции выброса левого желудочка, тяжелое поражение коронарных артерий и низкий уровень образования способствуют развитию УКР у пациентов с ишемической болезнью сердца. Предоперационное УКР не является основной причиной ПОКД, однако его наличие и развитие у этих пациентов ранней ПОКД способствует её сохранению через год после КШ.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To determine the incidence and risk factors of postoperative cognitive dysfunction (POCD) after coronary bypass (CABG) in patients with pre-operative moderate cognitive impairment (MCI).</p></sec><sec><title>Materials and Methods</title><p>Materials and Methods. Up to CABG two groups were formed, consisting of 50 patients with presence and 51 without MCI. Neuropsychological research (12 tests) was conducted up to CABG, on the 10th day and after 1 year. Early (10 days after surgery) and remote (1 year later) POCD were diagnosed with a 20 % decrease in the rate of 20% of tests.</p></sec><sec><title>Results</title><p>Results. Prior to CABG, they had a lower level of formation (p = 0.03), a lower fraction of the left ventricle ejection, (p = 0.02) and a more severe coronary tract lesion on the SYNTAX scale, (p = 0.01) than patients without MCI. Factors significant for the development of early POCD were the duration of with cardiopulmonary bypass (p = 0.035) and the severity of multi-organ insufficiency, rated on the SOFA scale (p = 0.04). In persons with pre-operative MCI, 72 % of cases were diagnosed with early POCD, 54 % with remote POCD, and 81 % with early POCD.</p></sec><sec><title>Conclusion</title><p>Conclusion. Chronic heart failure with low rates of left ventricular ejection fraction, severe coronary artery damage and low levels of education contribute to the development of MCI in patients with ischemic heart disease. Pre-operational MCI is not the main cause of POCD.
