Изучить типы вагинальных микробных сообществ у женщин с HSIL и NILM, проживающих в г. Самаре. Материалы и методы. В 2021–2023 гг. проведено изучение типов вагинальных микробных сообществ у 70 женщин в г. Самаре. В I группу (n = 40) вошли женщины с наличием цервикальных интраэпителиальных поражений тяжелой степени по результатам цитологического исследования, во II группу (n = 30) – с отсутствием предраковых поражений шейки матки. Пациентки обеих групп были инфицированы вирусом папилломы человека высокого канцерогенного риска, имели аномальную кольпоскопическую картину, репродуктивный возраст. Обследуемым было выполнено бактериологическое исследование отделяемого со слизистой оболочки цервикального канала с идентификацией выделенных микроорганизмов методом масс-спектрометрии. Результаты. Частота наличия нормального типа биоценоза при микроскопическом исследовании у женщин с HSIL была ниже в 5,3 раза, по сравнению с группой NILM (p = 0,023). Шансы выделения бактерий рода Streptococcus spp. в I группе в 3 раза выше, чем во II группе. У женщин с HSIL доминировал тип CST lV (75%), из подтипов преобладал CST lV-C 1 (превалировал Streptococcus) – 25% (p = 0,132). У пациенток с NILM доминировал также тип CST lV (60%), при этом самым многочисленным подтипом оказался CST lV-C 2 (превалировал Enterococcus) – 23,3% (p = 0,087). Частота наличия ВПЧ 16 типа в I группе в 3,9 раза выше, по сравнению со II группой (p = 0,008). АКК 2-й степени в I группе встречалась в 3,5 раза чаще, чем во II группе (p = 0,040). Заключение. Доминирующим типом среди микробных сообществ влагалища у женщин с HSIL и NILM был CST lV. В I группе из подтипов преобладал CST lV-C 1, во II группе – CST lV-C 2. Бактерии рода Streptococcus spp. значимо чаще обнаруживались у пациенток с HSIL.</p></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To study the types of vaginal microbial communities in women with high-grade squamous intraepithelial lesion (HSIL) and those negative for intraepithelial lesion or malig­nancy (NILM).</p></sec><sec><title>Materials and Methods</title><p>Materials and Methods. Between 2021 and 2023, we conducted an analysis of vaginal microbial com­munity types among 70 women of reproductive age infected with high-risk oncogenic HPV: those with HSIL verified by cytological examination (n = 40) and those without precancerous cervical lesions (i.e., NILM, n = 30). Identification of microorganisms in the cervical mucus was identified by mass spectrometry.</p></sec><sec><title>Results</title><p>Results. The frequency of normal microbiota detected by the microscopic examination was 5.3-fold lower, whilst Streptococcus spp. was detected 3-fold times higher in women with HSIL compared to the NILM group (p = 0.023). Among patients with HSIL, community state type (CST) IV was the most frequent (75%), and CST IV-C1 subtype (with Streptococcus spp. as a prevailing genus) was found in 25% of cases (p = 0.132). In patients with NILM, CST IV was also the predominant type (60%) but CST IV-C2 (with En­terococcus spp. as a prevailing genus) was most frequent subtype (23.3%, p = 0.087). The prevalence of HPV type 16 and grade 2 abnormal colposcopy findings were 3.9-fold (p = 0.008) and 3.5-fold (p = 0.040) higher in women with HSIL than in NILM (p = 0.008).</p></sec><sec><title>Conclusion</title><p>Conclusion.
