Роль клинических рекомендаций по профилактике и лечению сердечно-сосудистых заболеваний в снижении бремени болезней системы кровообращения

Цель. Оценить влияние внедрения во врачебную практику клинических рекомендаций (КР) по кардиологии в Российской Федерации (РФ) и некоторых странах Европейского региона на динамику смертности от сердечно-сосудистых заболеваний (ССЗ). Материалы и методы. Выборка данных о смертности от ССЗ была проведена с использованием базы данных Global Burden of Disease. Распределение стран на 5 кластеров по уровню смертности осуществлялось с помощью кластерного анализа. Информация об используемых КР была получена на сайтах профессиональных сообществ и по данным российских и зарубежных публикаций. Проведен эпидемиологический анализ смертности от ССЗ в периоды до и после внедрения КР. Проведены оценка динамики смертности и сравнение темпов прироста до и после внедрения КР. Результаты. После внедрения КР выявлено снижение смертности в Испании, Эстонии, России, Латвии, Республике Беларусь, Украине и более выраженный темп снижения в Великобритании и Нидерландах. Снижение смертности в Бельгии, Франции, Словакии, Италии, Португалии, Германии, Чехии обусловлено успешно реализуемыми кампаниями по профилактике ССЗ, внедрению и использованию КР в медицинской практике. Рост смертности в Хорватии, Сербии, Боснии и Герцеговине, Литве, Румынии и Болгарии связан с ограниченными экономическими возможностями, препятствующими полноценной реализации превентивных и лечебных программ. Заключение. Внедрение КР по ССЗ в систему здравоохранения сыграло важную роль в снижении смертности. Для достижения максимального эффекта от внедрения КР во врачебную практику необходима комплексная стратегия, обеспечивающая доступность качественной медицинской помощи для всего населения, в том числе программ профилактики и реабилитации, достаточный объем финансирования, включая современное оснащение медицинских организаций, подготовку и повышение уровня квалификации медицинских кадров. Лишь при условии интеграции КР в систему здравоохранения с обеспечением реализации изложенных в них направлений профилактики, диагностики и лечения можно достичь значительного снижения заболеваемости и смертности от ССЗ
