Матриксные металлопротеиназы. Клиническая и прогностическая значимость у больных инфарктом миокарда

Аннотация
Биологические маркеры часто используются в качестве суррогатных точек оценки риска, тяжести и прогноза заболевания. При этом не все из них формально являются потенциально мониторируемыми. Для наиболее оптимального использования биомаркеров в прогнозировании сердечно-сосудистого риска они должны отличаться не только точностью и воспроизводимостью при последовательных количественных измерениях, но и демонстрировать высокую чувствительность и специфичность. В последние годы наряду с классическими биомаркерами некроза миокарда, занявшими прочные позиции в клинике, появились новые - отражающие различные стороны развития патологического процесса при развитии инфаркта миокарда (ИМ): маркеры некроза (тро-понины), активации воспалительного процесса [(С-реактивный белок (СРБ), цитокины, матрикс-ные металлопротеиназы (ММП)] и маркеры миокардиальной дисфункции (предсердный и мозговой натрийуретические пептиды). При этом классические биомаркеры широко используются для своевременной диагностики ИМ, в то время как все большее число новых маркеров рассматриваются с позиции стратификации риска неблагоприятных исходов у больных острым коронарным синдромом (ОКС).
